Musculoskeletal Pain Part II: NeuralProlotherapy

Dr Susan Janssens, BSc, ND

Hi everyone, in part I of this series I wrote about Neural Therapy if you were not able to catch the article please go to my blog at www.IHConline.ca. I left off with having worked on scar tissue and interference fields for my chronic back pain. The next pain therapy that I decided to study was an injection technique called Neural Prolotherapy; developed by John Lyftogt, MD, an accomplished sports medicine doctor in New Zealand.

This injection therapy targets the nerves that control your muscles. When nerves become injured they are thought to be responsible for painful conditions described as ‘neuralgias’ or ‘neuropathic pain’ or more commonly known as ‘chronic pain’. Often this type of pain differs from other forms of inflammation in that it does not respond well to anti-inflammatory drugs or cortisone injections and this is the reason why these commonly used drugs are proving to be ineffective in many painful conditions. In addition, there is a growing awareness that cortisone and anti-inflammatory drugs are associated with a variety of harmful side effects.

The protective sheath of the nerve trunk is structurally very similar to tendons and ligaments. It protects small nerve fibers inside the nerve trunks from injury, friction and pressure.  Think of nerves to be like electrical wires…what happens to an electrical circuit if a wire damages its protective coat…perhaps your light won’t turn on or worse you will get an electric shock or fire.  If your nerves are not firing properly your muscles will go into spasm and not work as well as they should.

Dr Liftogt states, “It is clear from clinical observations on more than five thousand patients and large case series that Neural Prolotherapy effectively reverses ‘neurogenic inflammation’ and resolves ‘neuralgia’ and ‘neuropathic’ or chronic pain.”

Going back to my own injuries; though my back muscle was much more flexible (I can now touch my toes) and I was in much less pain after the neural therapy. I would still occasionally have episodes where I would twist in a certain way and I would be in immediate and debilitating pain that would lay me up for up to a week or two. I knew I was still missing a piece of the puzzle.  As it happened, I picked up my luggage with a little twist and was in one of these episodes when I went for training for NPT. I offered myself to the class and Dr Liftgott was able to palpate the inflamed nerves and inject 5% mannitol subcutaneously around them.  After the first injection I was able to get off the table and walk unassisted and by day three was pain free. He said that I had such a complex back that I would most likely require 8 injections but I was done after five.

So what is the success rate of NPT? Depending upon the body part, how long you have had the injury/pain for, and the individual circumstances, the “rule of thumb” is that 85% of people resolve their pain completely within 6-8 treatments, with many more experiencing a significant improvement. I have treated knees, backs, shoulders, neuromas, neck and facial nerves with similar success rates. As a Naturopathic Doctor I choose to use natural therapies as often as possible. If I am able to help someone to reduce or stop using pain medications and to live a full life, I have done my job.  Often, I must tell my patients that it took years to develop a condition, so it will possibly take considerable time to resolve it. With NPT, this does not have to be the case; patients and clinicians are greatly encouraged by immediate results.
